I presume you're on Windows, probably 2000 or XP. If this isn't the case then pls let correct me. If so then the problem is with Direct3D.
There's a few things you can try. One is to go into Control Prefs, look over the right hand side of the screen and you'll see DirectInput. If its checked then uncheck it, or if its unchecked then check it. See if that helps.
If no luck, then you could try this Glide Wrapper - http://www.zeckensack.de/glide/ - install it and when you run Myth again you'll see an option for 3DFX. Use that instead of Direct3D.
If you're still not getting anywhere, a public beta of 1.5.1 will be out very Soonâ„¢, and features OpenGL on Windows.
